If your consent is needed in the first place, you can change it or withdraw it. To do that, you need to give your health care provider a clear written instruction.   If you need help putting your instructions into writing, your health care provider is required to help you do that.  

Remember, your consent is not always needed, especially for certain types of disclosures. Sometimes your health care provider is required by law to disclose your health information, even if you tell him or her that you don't consent.
